You should have a look at the sample codes.

dMatrix3 R;
dRFromAxisAndAngle(R, 0.0, 0.0, 1.0, angle);

and there are a few other such functions too.

The matrix is a 3x4 matrix and last time I checked,
dRFromAxisAndAngle didn't set the last column but that
isn't a problem.
