[ODE] ODE Message 3?

To whom it may concern,

In my most recent ODE model, I have encountered the following ODE error 
during simulation.

ODE Message 3: LCP internal error, s <= 0 (s = <num>)

What is causing this error?  What are the typical causes?  And are there any 
fixes for it?
