[ODE] Stepfast

A patch that'll take you from a recent-ish CVS HEAD
to a Stepfast-enabled ODE is available at:

These include David Whittaker's last batch of Stepfast docs
in HTML format.  You'll need to read these (and/or look at the
new test_crash demo) to be able to use Stepfast properly.  Note that
the docs refer to all functions postfixed with '1' (i.e.
dWorldStepFast() becomes dWorldStepFast1()) because of a mismatch
between the up-to-dateness of the last-released code and the
last-released docs for Stepfast, but the documentation is otherwise
pertinent.

Note that I didn't write this code, I'm merely repackaging it
in a form that's easily applicable to recent CVS ODE versions.
